Understanding Flashes, Floaters and Changes in Your Vision

A Posterior Vitreous Detachment (PVD) is a common age-related change inside the eye. Although the name sounds alarming, it is usually a normal part of ageing and, in most cases, does not threaten your vision.

However, because a PVD can occasionally lead to a retinal tear or retinal detachment, it is important to have your eyes examined promptly if you notice a sudden onset of floaters or flashes of light.

What Is a Posterior Vitreous Detachment?

The inside of the eye is filled with a clear, jelly-like substance called the vitreous. When we are young, the vitreous is firmly attached to the retina, the light-sensitive tissue lining the back of the eye.

As we age, the vitreous gradually becomes more liquid and begins to shrink. Eventually, it separates from the retina. This natural separation is known as a Posterior Vitreous Detachment (PVD).

A PVD is extremely common and is considered a normal part of the ageing process.

What Causes a PVD?

Most PVDs occur naturally with age, but they may develop earlier in people who:

  • Are short-sighted (myopic)

  • Have had cataract surgery

  • Have experienced an eye injury

  • Have inflammation inside the eye

  • Have undergone previous eye surgery

Most people develop a PVD sometime after the age of 50.

Common Symptoms

Many people notice symptoms suddenly over a period of hours or days.

Symptoms may include:

  • New floaters

  • Flashes of light

  • Cobwebs or strands drifting across your vision

  • Small black dots

  • A large ring-shaped floater

  • Blurred vision caused by floaters

The floaters often become less noticeable over several weeks or months as the brain gradually adapts.

Why Are Flashes of Light Seen?

When the vitreous separates from the retina, it may gently tug on the retina during the process.

This mechanical stimulation causes you to see brief flashes of light, particularly in dim lighting or when moving your eyes quickly.

The flashes usually become less frequent as the vitreous completes its separation.

Can a PVD Cause Retinal Detachment?

Most Posterior Vitreous Detachments do not cause permanent problems.

However, in a small number of patients, the vitreous can pull hard enough to create a retinal tear. If untreated, fluid may pass through the tear and lead to a retinal detachment, which is a sight-threatening emergency.

This is why every new onset of flashes or floaters should be assessed by an ophthalmologist.

Warning Signs That Require Urgent Attention

Seek urgent eye care if you experience:

🚨 A sudden shower of new floaters

🚨 Frequent flashes of light

🚨 A curtain or shadow moving across your vision

🚨 Loss of side vision

🚨 Sudden reduction in vision

🚨 A rapid increase in floaters

These symptoms may indicate a retinal tear or retinal detachment and should never be ignored.

How Is a PVD Diagnosed?

Dr Cheryl Lee performs a comprehensive dilated eye examination to examine the vitreous and retina.

The examination looks for:

  • Posterior Vitreous Detachment

  • Retinal tears

  • Retinal holes

  • Retinal detachment

  • Vitreous haemorrhage

Even if your retina appears healthy initially, a follow-up examination may sometimes be recommended because retinal tears can occasionally develop after the first visit.

Treatment

A Posterior Vitreous Detachment itself usually does not require treatment.

Most floaters become less noticeable over time as they settle lower within the eye and the brain gradually learns to ignore them.

If a retinal tear is detected, prompt laser treatment may be recommended to reduce the risk of retinal detachment.

If a retinal detachment develops, surgery is usually required.

Can a PVD Be Prevented?

Unfortunately, no.

A Posterior Vitreous Detachment is a natural part of ageing and cannot usually be prevented.

The most important step is recognising the warning signs and seeking prompt examination if symptoms develop.

One of the most common concerns I hear is, "Doctor, am I going blind?"

Fortunately, the answer is usually no. Most Posterior Vitreous Detachments are a normal part of ageing and do not threaten vision.

The important point is not to ignore a sudden onset of floaters or flashes. A careful examination of the retina allows us to distinguish a harmless Posterior Vitreous Detachment from the small number of patients who have developed a retinal tear or retinal detachment.

Early diagnosis is the key to protecting your vision.

New floaters deserve an eye examination—not because they are usually serious, but because occasionally they are.

Is a Posterior Vitreous Detachment an emergency?

A PVD itself is usually not an emergency, but because it can occasionally cause a retinal tear, anyone with new floaters or flashes should have an urgent eye examination.

Will my floaters disappear?

Most floaters become less noticeable over time. While they may not disappear completely, many people find that they interfere much less with vision after several months.

Can I drive with a PVD?

Most people can continue driving if their vision meets legal requirements. However, if you develop sudden vision loss or a curtain across your vision, seek urgent medical attention and avoid driving until you have been assessed.

Will it happen in my other eye?

Yes. Many people eventually develop a Posterior Vitreous Detachment in the other eye, although this may occur months or years later.
